• Randall Residence, 6400 S Co Rd 25A, Tipp City: Standard/Critical Control Point Inspection.

Corrected During Inspection; Critical; Repeat; At time of inspection, observed can opener with dirt and debris build up. After informing PIC, the can opener was pulled for cleaning.

At time of inspection, observed reduced oxygen packaged (ROP) fish thawing without puncturing the packaging. Informed PIC that the ROP must be punctured before thawing. After informing PIC, thawed fish that was not punctured was voluntarily discarded.

Corrected During Inspection; Critical; At time of inspection, observed food containers with cracks and severe discoloration. After informing PIC, the containers were discarded.

At time of inspection, observed the dishwasher not properly sealed to the wall. Reseal dishwasher to the wall.

Repeat; At time of inspection, observed marred cutting boards. Resurface or replace cutting boards that are excessively marred.

At time of inspection, observed the following: 1. Inside juice machine observed with sticky build up. Clean inside juice machine. 2. Inside grill prep coolers observed with debris build up. Clean inside grill prep coolers. 3. Mold like build up on caulking around hand sink in main kitchen. Clean around hand sink.

• Randall Residence Memory Care Kitchen, 6400 S Co Rd 25A, Tipp City: Standard Inspection.

At time of inspection, observed the following: 1. Utensil storage container to have debris. Clean utensil storage containers. 2. Microwave inside observed with food debris. Clean microwave. 3. Reach-in cooler observed with sticky juice build up and various debris, and with dirty gaskets. Clean out reach-in cooler and clean gaskets. 4. Freezer observed with ice build up. De-ice freezer.

• Smith’s Boathouse Restaurant, 439 N Elm St, Troy: Follow-Up Inspection.

No violations were documented at the time of inspection.

The following violation(s) have been corrected since the last inspection:

Backflow prevention device – when required – No approved backflow prevention device on the mop sink. Implement a vacuum breaker. Due to continuous flow of water, an additional backflow will need to be installed on the spigot as well.

Comments:

Upon re-inspection, observed a backflow device on the mop sink.

• Dominos Pizza, 937 W Main St, Troy: Standard Inspection.

No written procedures for responding to vomiting or diarrheal events.

Personal care items stored improperly. Observed employee food being stored on the top shelf of the reach in cooler. Upon informing the person in charge, the food was moved.

Improper use of sponges. Observed sponges in the facility being used to wash pans. Ensure that sponges are not used on food contact surfaces.

Equipment and utensils are not being air dried. Observed containers that were still wet and stacked on the clean dish rack.

Observed pizza boxes being stored on the ground. Ensure single-service items are stored at least 6 inches off the ground. Upon informing the person in charge, the boxes were moved.

Fixed equipment not properly sealed or spaced for cleaning. Observed the 3 compartment sink and the hand sink not properly sealed to the wall.

Equipment components are not intact, tight or properly adjusted. Observed the gaskets on the reach in prep cooler in disrepair.

Repeat; Observed tape on the reach in prep cooler doors causing the nonfood-contact surface to no longer be smooth and easily cleanable.

Repeat; Non-food contact surfaces of equipment are unclean. Observed food debris at the bottom of the reach in cooler.

Repeat; Physical facilities not maintained in good repair. At time of inspection, floor tiles were observed missing throughout the facility.

Repeat; Facility not maintained clean. Observed a mold like substance on the wall behind the 3 compartment sink.

Repeat; Ventilation system not maintained. The hood vents above the pizza oven were observed unclean with dust and grease build up.

Repeat; RFE does not comply with Level One Certification requirements.

Comments:

At time of inspection, an individual certified in food safety was not available. Ensure there is at least one individual with a person in charge certification per each shift.

• McDonalds, 1560 W Main St, Troy: Standard/Critical Control Point Inspection.

Critical; PIC unable to demonstrate knowledge of cleaning and sanitizing. At time of inspection, Person-In-Charge (PIC) was unable to state correct cleaning procedures for soda nozzles and frappe machine.

Corrected During Inspection; Critical; Quaternary ammonium sanitizing solution at incorrect temperature. Observed sanitizing solution at 124F. Upon informing the PIC, the sanitizing solution was re-made at 75F or below.

Personal items stored improperly. Observed employee food stored directly next to food to be used in the facility in the walk-in cooler.

Observed several working food containers not properly labeled. Label accordingly.

Observed utensils stored directly above the prep sink, exposed to splash contamination. Re-locate utensils to prevent contamination.

Observed equipment and utensils are not being air dried in the warewash area.

Fixed equipment not properly sealed or spaced for cleaning. Observed escutcheon plates and gap in between PVC and wall underneath hand-sink across from the drive thru. Eliminate gaps and seal so that surfaces are smooth and easily cleanable.

Repeat; Non-food contact surfaces of equipment are unclean. Observed food residue and grime on storage racks, interior and exterior surfaces of equipment such as reach-in coolers, fryers, and prep tables, including the gaskets and door handles. Clean.

Physical facilities not maintained in good repair. Observed grout in the warewash area in poor condition with standing water. Repair.

Repeat; Facility not maintained clean. Observed food residue and grime throughout the facility including floors, walls, ceilings, behind equipment in hard to reach areas, and the walk-in fans with a large accumulation of dust. Also observed walk-in freezer floor with heavy dirt build-up. Clean at a frequency necessary to prevent such accumulations.

Comments:

Ensure all equipment is being cleaned per manufacturer’s instructions and as needed.

• MU Bulldog Diner, 30 Lowry Dr, West Milton: Follow-Up Inspection.

No violations were documented at the time of inspection.

The following violation(s) have been corrected since the last inspection:

Cleanliness of equipment food-contact surfaces and utensils – Equipment food-contact surfaces or utensils are unclean. Observed a mold like build up on the ice machine.

Repairing – Physical facilities not maintained in good repair. At time of inspection, the walk in freezer was not maintaining temperature and the food inside was starting to thaw.

Comments:

At time of inspection, the freezer was observed working and the food was frozen. The ice machine was observed clean and build up was removed.

• Springmeade Health Center, 4375 S Co Rd 25A, Tipp City: Follow-Up Inspection.

No violations were documented at the time of inspection.

The following violation(s) have been corrected since the last inspection:

Backflow prevention – air gap – At time of inspection, observed the following: 1. Air gap under 2-bay prep sink to be insufficient. An air gap of 2 – 4 inches or twice the diameter of the pipe must be created. 2. Copper piping from the dishwasher observed to be not properly air gapped. Ensure piping is properly anchored and an air gap of 2 – 4 inches is created.
