Information provided by Miami County Public Health.

Jan. 19

• Troy Bowl, 1530 McKaig Ave., Troy: Follow-Up Inspection.

Repeat: Facility does not have an employee with manager certification in food protection.

Repeat: Facility not maintained clean. Floors, walls, and ceilings throughout observed with dust, dirt, and food debris.

Repeat: FSO did not have a person in charge per shift with the person in charge certification in food protection.

The following violation(s) have been corrected since the last inspection:

Mechanical warewashing equipment – hot water sanitization temperatures: Hot water for mechanical sanitization is below required temperature. PIC stated ware washing machine not functioning at the time of inspection.

Comments:

At time of follow-up inspection, the dish machine was fixed and properly functioning.

• Troy Country Club, 1830 Peters Rd., Troy: Standard/Critical Control Point Inspection.

Critical: Person-in-charge (PIC) unable to demonstrate knowledge by having no critical violations.

Repeat: Employee eating, drinking, or using tobacco in non-designated area. Observed an employee drink on the prep line. An employee phone was observed on a cutting board on the prep line. Upon informing the employee, the phone and drink were moved.

Corrected during inspection; critical: Food employee(s) did not wash hands when required. Observed an employee with gloved hands touch a spray bottle containing chemicals and then proceed to go back to prepping food without washing hands between. Upon informing employee, hands were washed and donned with new gloves.

Corrected during inspection; critical: Handwashing sink is being used for purposes other than handwashing. Observed hand washing sink at the bar being used as a dump sink. Discussed with PIC other options to have a dump area in the bar.

Corrected during inspection; critical; repeat: Food not properly protected from contamination by separation, packaging, and segregation. Observed ready to eat French onion soup being stored under raw salmon and next to raw chicken. Upon informing PIC, the soup was moved to the top shelf.

Corrected during inspection; critical: Improper reheating of food for hot holding. Observed bourbon pecan sauce being reheated and did not reach 165F within the 2 hour time period. PIC discarded this batch of the sauce and reheated a different batch to 165F.

Corrected during inspection; critical: Improper use of time as a public health control (4 hours). Observed butter setting out at room temperature longer than 4 hours. Upon informing PIC butter was discarded.

Corrected during inspection; critical: Working containers of poisonous or toxic materials not properly labeled. Observed a spray bottle with an unknown substance and no label. Upon informing PIC the bottle was labeled as sanitizer.

Repeat: TCS foods not properly thawed. Observed ROP fish being thawed in the walk in cooler without the packaging being sliced open. Informed PIC that packaging needs to be cut opened when thawing under refrigeration or sealed under cold running water and opened after thawing. PIC discarded the fish that was thawing without being cut.

Food not protected from contamination. Observed air vents over prep areas with a severe build up of dust.

Irreversible registering temperature indicator not readily accessible for hot water mechanical warewashing operations. PIC could not locate test strips for warewash machine at time of inspection.

Repeat: Non-food contact surface(s) not cleaned at the required frequency. Shelving racks in the walk in cooler were observed with a food debris build up.

• Hot Head Burrito – Troy, 1287 S. Dorset Rd., Troy: Follow-Up Inspection.

Repeat: Facility not maintained clean. Floors throughout the facility, including inside the walk-in cooler were observed with food debris. Walls behind the back prep table observed with food debris and splatter.

The following violation(s) have been corrected since the last inspection:

Controlling pests: Presence of live insects, rodents, and other pests. A gnat presence was observed during the inspection.

Comments: At time of follow up inspection, PIC provided an invoice from pest control. A minimal number of gnats are still present. Keep working with a licensed pest control company to continue to control the pest.

Improvement of pest control will be checked at next inspection.

Jan. 20

• 1314 East Side Sunoco Inc., 1314 E. Main St., Troy: Standard Inspection.

No violations were documented at the time of inspection.

Comments: Use the pre-licensing report to see items needing completion for the change in ownership at 1314 East Side Sunoco.

Begin to work on these items to bring the facility up to code and they will be re-checked at a later date to ensure compliance is being attained with the Ohio Uniform Food Safety Code.

As mentioned in the pre-license report, prior to installation or work beginning on the walk-in cooler project, submit a plan to the Health Department for full review and approval.

• Indian’s Pizza, 212 N. Main St., Pleasant Hill: Follow-Up Inspection.

No violations were documented at the time of inspection.

The following violation(s) have been corrected since the last inspection:

Backflow prevention – air gap: The prep sink was observed without an air gap between the drain pipe and drain. To prevent contamination of the sink, provide a two to four inch air gap between the drain pipe and drain.

Comments: Reinspection was conducted due to report dated 01/21/2022. On 1/22/2022, the owner provided a picture of what appeared to be an adequate air gap between the prep sink drain pipe and drain. Because of this, the violation was corrected and satisfactory. The other violations and sealing of sinks and catch basin beneath the shelving unit will be checked during the next standard inspection.

• Bowman & Landes Turkeys Inc, 6490 E. Ross Rd., New Carlisle: Follow-Up Inspection.

No violations were documented at the time of inspection.

The following violation(s) have been corrected since the last inspection:

Food-contact surfaces – cleanability: Food contact surfaces not easily cleanable. Food contact surfaces not easily cleanable. At time of inspection, observed meat tenderizer cover to have a large crack in it. The surface is no longer smooth and easily cleanable and needs to be replaced.

Comments: Meat tenderizer cover replaced, and picture received via email (picture attached below). A reinspect did not occur on-site.
