• Subway, 1281 S. Dorset Road, Troy — Critical; Repeat: Multiple plastic food storage containers and lids were observed with cracks or surfaces missing. Replace any food storage containers and lids that are in disrepair.

Critical; Repeat: On the tea and lemonade bubbler system in the dining room, a backflow preventer was not observed. Install a backflow preventer on the dining room bubble system.

Critical: The drive-thru and dining room soda fountain machine ice makers drain lines were observed without air gaps. Place a 2- to 4- inch air gap between the drain lines and drains.

Repeat: At the time of inspection, the establishment was without an individual on hand who had Level One (Person-in-Charge) Food Safety Training. Have an individual on hand at all times who is at least Level One Food Safety Trained.

• Early Beginnings Child Center, 1021 S. Dorset Drive, Troy — No violations were documented at the time of inspection.

• Outback Steakhouse, 1801 Towne Park, Troy — Critical: Chlorine sanitizing solution at incorrect temperature and/or concentration. Observed the single-rack dish machine behind the bar was not able to provide chlorine sanitizer of at least 50PPM. PIC replaced sanitizer jug and machine is now providing 100PPM chlorine concentration.

Facility not maintained clean. Observed residual food debris and dust/dirt accumulation throughout facility. Increase cleaning frequency to prevent these accumulations in the following areas: Under shelving in dry storage; Shelving and under shelving in the walk-in cooler and freezer; Shelving used to store clean dishes; Light fixtures in walk-in cooler; Vent covers above prep sink; Vent of salad cooler cabinet and wall behind; Any area in the facility not identified on this report.

• McDonald’s, 1560 W. Main St., Troy — Employee eating, drinking, or using tobacco in non-designated area. Observed employee beverage stored on shelving above three-compartment sink while dishes were being done. PIC voluntarily discarded

Floors, walls, and/or ceilings not smooth and easily cleanable. Observed missing ceiling tile in the back tool storage area, where the automatic sprinkler pipe enters the ceiling; replace. Also observed small hole in ceiling tile above the new water heater; replace. Observed a hole in the wall behind the fountain soda boxes in the back by the mop sink. This hole has a valve inside the wall that the facility needs access to. It appears there used to be a closable door covering this hole. Either seal up hole or replace closable door with latch.

Repeat: Facility not maintained clean. Observed substantial amount of residual food debris build-up in the following areas. Increase cleaning frequency to prevent these accumulations: Behind fountain soda boxes in the back by the mop sink; In between the two oil tanks in the back dry storage area by the side door; The walk-in freezer floor; The floor in the chemical closet; Under the fountain soda dispenser in the lobby, underneath the pressurized ketchup dispenser.

• Certified, 891 S. Miami St., West Milton — No violations were documented at the time of inspection.

Jan. 8

• River Rock Bar and Grill, 5795 N. County Road 25A, Piqua — Corrected During Inspection; Critical: In the walk-in cooler, raw eggs were observed being stored directly on top of deli turkey. Upon making the PIC aware of this, the PIC separated the eggs from the deli turkey.

Corrected During Inspection; Critical: In the hot holding unit adjacent to the convection oven, a container of tomato soup was observed holding at 110° F. Upon making the PIC aware of this, the PIC began the process of reheating the soup.

Corrected During Inspection; Critical: In the front section of the kitchen, toppings being held on ice were observed with temperatures ranging from 51° F to 55° F. Upon making the PIC aware of this, the PIC placed the sauces in the reach-in freezer.

Corrected During Inspection: In the cold holding drawers below the hard top grill, four reduced oxygen packages of salmon were observed completely thawed while still in the ROP environment. To prevent growth, fish in a vacuum sealed environment shall be opened prior thawing under refrigeration or, if thawing under cool running water, shall be immediately opened after thawing. Upon making the PIC aware of this, the PIC discarded the four packages of salmon.

Corrected During Inspection: In the back kitchen area, boxes of raw meat were observed being thawed at room temperature. Upon making the PIC aware of this, the PIC placed the raw meat in the walk-in cooler.

Multiple glasses in the waitress area and food storage pans in the kitchen were observed being stored on top of each other while wet. Ensure they are completely air dried prior to stacking.

The True reach-in cooler adjacent to the mop sink was observed with door gaskets in disrepair. Replace.

The shelving units in the True reach-in cooler adjacent to the mop sink were observed with a black, mold-like substance build-up. Remove items and clean.

• Kroger, 731 W. Market St., Troy — Corrected During Inspection; Critical; Repeat: Date the last shellstock was sold or served was not recorded on the label. Observed shellstock tags that were not denoted with the date that the last shellstock from that specific batch was sold or served. This issue was discussed with PIC and department head. They will start recording proper date on tags.

Critical; Repeat: Utensils and food-contact surfaces of equipment not sanitized at the required frequency. Observed conditioning tubs in the produce cooler that were not cleaned and sanitized after/before each use. The tubs were observed with residual food/ dirt accumulations at the corners and bottoms of the tubs. PIC was able to wash rinse/sanitize conditioning tubs in use at time of inspection and has been instructed to clean/sanitize the tubs after each use.

Corrected During Inspection; Critical: White rice improperly acidified to render it non-TCS, or without a HACCP plan. Observed pH log was not completed on the day of inspection and various other dates throughout the last few months. Each day these logs must be completed. PIC completed log during inspection.

Plumbing system not properly maintained or repaired. Observed small leak in prep sink of Bistro Kitchen. Repair according to plumbing code.

• End Zone Sports Lounge, 601 E. Broadway Ave., Covington — Critical; Repeat: At the time of inspection, the establishment was without an employee health policy and in a verifiable manner did not inform employees their responsibility to report their health information.

Repeat: At the time of inspection, the establishment was without a bodily fluid clean-up procedure. Obtain a bodily fluid cleanup procedure and ensure items in the procedure are on hand.

Corrected During Inspection; Critical: The deli slicer and cuber were observed with food debris build-up. Upon making the PIC aware of this, the PIC placed the cuber blade in the dirty dish area and cleaned the surrounding surfaces. The PIC stated the deli slicer will be broken down and cleaned and sanitized.

Corrected During Inspection; Critical: One container of turkey and one container of mashed potatoes were observed without date markings. Upon making the PIC aware of this, the PIC discarded the turkey and mashed potatoes.

Corrected During Inspection; Critical: In the sandwich prep cooler, small containers of sauces were observed past their use-by-date. Upon making the PIC aware of this, the PIC discarded the small containers of sauces.

Corrected During Inspection; Critical: In the walk-in cooler, one container of chili was observed with a date marking of 12/28. Upon making the PIC aware of this, the PIC discarded the chili.

Corrected During Inspection; Critical: Above the three-compartment sink, four bottles of cleaning chemicals were observed being stored. Upon making the PIC aware of this, the PIC moved the chemicals to another location.

Multi-use eating utensils in the holding area were observed being stored in a sorter with food and dust debris build-up.

Repeat: In the furnace room, boxes of single-serve items were observed being stored on the floor. Place items at least 6 inches off the floor.

Critical: Multiple Sterilite containers were observed being used to store food items. Cease the use of Sterilite containers for direct food storage.

Critical: Sour cream containers were observed being reused to store in-store-made food items. Cease use of sour cream containers for any direct food contact storage.

Surfaces of cabinets and drawers, along with bottom shelves of tables throughout the kitchen, were observed with food debris build-up.

• Covington New Pre-K through 8 School, 807 Chestnut St., Covington — No violations were documented at the time of inspection.

• Buffalo Jack’s, 137 S. High St., Covington — Repeat: In the upstairs storage area, unnecessary items and litter were observed. Because of the unnecessary items, harborage areas for pests were provided. Remove the unnecessary items and litter.

Jan. 9

• Tropical Smoothie Cafe, 2309 W. Main St., Troy — No violations were documented at the time of inspection.

• Laura’s Country Diner, 6 W. Pike St., Laura — Repeat: At the time of inspection, the establishment was without an individual who had Level Two (Managerial) Food Safety Certification. PIC stated class is scheduled to be taken in February.

Critical: One layer of eggs were observed holding at room temperature. Eggs shall be maintained at or below 45° F or time stamped. If using time, develop a procedure, time the eggs and then discard after four hours.

Corrected During Inspection; Critical; Repeat: At the time of inspection, the menu was observed without a consumer advisory for made-to-order eggs. PIC stated the new menu will have a consumer advisory on it.

Food items in the walk-in cooler were observed without a covering or lid. To prevent contamination from above, place a wrapping or covering on the containers of food.

Repeat: In the walk-in cooler, boxes of raw eggs were observed being stored on the floor. Place at least 6 inches off the floor.

Repeat: Cooking and serving utensils, along with knives, were observed being stored in containers with food debris build-up. Remove utensils and clean and sanitize storage containers. Also, reclean and resanitize utensils.

The bottom shelving of prep tables, the shelving units throughout the kitchen and the handles of equipment were observed with dust and food debris build-up. Remove items and clean.

Repeat: At the time of inspection, the establishment was without an individual who is certified in Level One (PIC) Food Safety. Have an individual on hand at all times who is at least Level One (PIC) Food Safety certified.

• American Legion #586, 377 N. Third St., Tipp City — No violations were documented at the time of inspection.

• Broadway Elementary School, 223 W. Broadway St., Tipp City — No violations were documented at the time of inspection.

• Chin’s Ginger Grill, 965 W. Main St., Tipp City — Repeat: Employee eating, drinking, or using tobacco in non-designated area. Observed unapproved employee beverage containers (no lids) stored on the prep table in the kitchen and on the shelf above the prep top cooler. Ensure employees eat and drink in designated areas only and store their food and drinks in a manner that cannot contaminate clean equipment, utensils, or food.

Repeat: Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed cutting boards on prep top coolers scored, chipped, and no longer smooth and easily cleanable. Resurface or replace.

Critical; Repeat: Insufficient air gap between the flood rim and the water supply inlet. Observed insufficient air gap on prep sink in kitchen.

Repeat: Physical facilities not maintained in good repair. Observed the wall behind the ice machine needs to be repaired.

Repeat: Floors, walls, and/or ceilings not smooth and easily cleanable. Observed missing ceiling tiles in kitchen. Replace.

Repeat: Facility not maintained clean. Observed residual food and dust/dirt accumulation throughout facility underneath equipment and shelving, on walls and floors. Cleaning needs to be increased in the walk-in cooler, under reach-in coolers and freezers in kitchen, behind grill/wok line, under the hood above the grill/wok line, air returns in kitchen ceiling, around dish machine and three-compartment sink.

• Tipp City Eagles, 202 Main St., Tipp City — No violations were documented at the time of inspection.

• Mojo’s Bar & Grill, 109 E. Main St., Troy — Repeat: Multi-use items in the basement were observed uncovered and beneath drain lines. Cover and move items away from drain lines. During the inspection, did observe some multi-use items in plastic containers.

Multi-use food containers above the three-compartment sink were observed wet being stored on top of each other. Ensure containers are completely dried prior to storing.

Critical: Plastic food containers were observed with cracks and section missing. Replace the plastic food containers in disrepair.

• Cassano’s, 1201 Experiment Farm Road, Troy — The walk-in cooler door gasket was observed in disrepair. Replace.

At the time of inspection, the establishment was without quaternary ammonium sanitizer test kit. Obtain quat test strips.

The following non-food contact surfaces were observed with dust or food debris build-up: 1. The dry good shelving units 2. The interior surfaces of the reach-in coolers (including shelving units) 3. The exterior surfaces of the reach-in coolers 4. The door gaskets on the walk-in and reach-in coolers 5. The drawers below the pizza cutting surface 6. The bottom shelf of the prep tables — Remove items and clean.

The following physical facilities were observed with dirt and dust debris build-up: 1. The HVAC vents and adjacent ceiling tiles 2. The floors under pieces of equipment and shelving units 3. In corners of kitchen.

Jan. 10

• Jonesy’s Pizza, 1469 S. County Road 25A, Troy — No violations were documented at the time of inspection.

• Brookdale, 81 S. Stanfield Road, Troy — Corrected During Inspection; Critical: Multiple tongs and a spatula were observed with food debris build-up. Upon making the PIC aware of this, the PIC placed the utensils in the dirty dish area.

The door gasket on the reach-in freezer adjacent to the fryers was observed in disrepair. Replace.

• Leaf & Vine, 108 W. Main St., Troy — Corrected During Inspection; Critical: Small containers of sauces in the bar low boy cooler were observed past their use-by-date. At the time of inspection, the PIC discarded the small containers of sauces.

Corrected During Inspection: Food items in the basement freezer were observed unthawed. Upon showing the PIC this, the PIC decreased the temperature of the freezer.

Repeat: At the time of inspection, the establishment was without an individual who was Level One Food Safety Certification (PIC). Have an individual on hand at all times who is Level One or Level Two (Managerial) Food Safety Certified. For on-line Level One training, use ServSafe.com or StateFoodSafety.com. Ensure the Food Handlers Course is taken.

• Coldwater Cafe, 19 E. Main St., Tipp City — Outer opening not protected. Observed substantial gap between the back double doors of the Bodega side of facility. Seal outer openings to prevent pest intrusion. No light should be seen around perimeter.

• Guadalajara’s Mexican Restaurant, 301 E. Troy Pike, Suite C, Covington — Repeat: Handwashing sink water below 100°F. Observed handsinks throughout the facility were only able to provide water of 96°F during inspection. Hand sinks must provide water of at least 100°F for hand washing.

Repeat: Outer opening not protected. Observed substantial gaps around the perimeter of the back door to facility. Also observed substantial gaps around the perimeter of the doors in the unused warehouse side of the facility.

Repeat: In-use utensils improperly stored. Observed knives stored in the gap between the two prep top coolers.

Repeat: Clean equipment and utensils not stored in a self-draining position and covered or inverted. Observed clean forks, knives, and spoons stored with the biting surface side up. Store inverted once cleaned and sanitized.

Repeat: Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed cutting boards scratched, scored, and chipped and no longer smooth and easily cleanable.

Repeat: Floors, walls, and/or ceilings not smooth and easily cleanable. Observed missing floor tiles in the kitchen in front of the ice machine, under the table where chips and salsa are stored, and observed holes in the walls of the kitchen in front of the chips and salsa area, in the wall by the bar, and in the wall behind the grill line.

Repeat: Facility not maintained clean. Observed residual food debris and dust/dirt accumulation under most of the bar area, in the kitchen under the shelving and fixed equipment, and behind the grill line. Increase cleaning frequency to prevent these accumulations.

Repeat: Physical facilities not maintained in good repair. Observed in the back, by the mop sink the the wall is water damaged and deteriorating. Observed mold growth insisde of the wall cavity by the mop sink.

Repeat: Mops dried improperly. Observed mops stored in mop buckets with old stagnant, putrid water in the back by the mop sink.

Repeat: FSO did not have a person in charge per shift with the person in charge certification in food protection. Observed no Level 1 Food Safety Certification in facility.

• The Caroline, 5 S. Market St., Troy — No violations were documented at the time of inspection.

• Harrison’s, 106 E. Main St., Tipp City — Corrected During Inspection; Critical: Food not properly protected from contamination by separation, packaging, and segregation. Observed ground beef hamburger patties stored in the walk-in cooler, above pork and whole muscle intact beef. PIC moved ground beef below pork and muscle intact beef.

Critical; Repeat: Equipment food-contact surfaces or utensils are unclean. Observed mold growth on the inside of the ice machine. Increase cleaning frequency to prevent these accumulations.

Improper use of a rodent bait station. Observed unapproved rodent bait station in use in basement. Bait station was Protecta RTU. Self contained, tamper proof bait stations are the only bait stations approved for use in Food Service Operations. PIC voluntarily discarded.

Critical: Presence of live insects, rodents, and other pests. Observed mouse droppings underneath shelving in the basement. Mostly in the corners underneath shelving where dry food storage is located. Use wet cleaning methods to removed mouse droppings, sanitize the area, and monitor for new activity.

Facility not maintained clean. Observed residual food debris and dust/dirt accumulation throughout facility, mostly concentrated in the kitchen. Increase cleaning frequency to prevent these accumulations. The following areas need to be cleaned: -The floors/walls/ceilings that are under/behind/above all fixed equipment in the kitchen area. Large accumulations of grease and food debris was observed underneath coolers, dish machine, ice machine, etc. -Shelving throughout facility including in the walk-in cooler and freezer -The handles of most of the coolers, including reach-in coolers and walk-in coolers. -The floor of the walk in freezer. -Underneath shelving in the basement. -The phone in the kitchen and the printer beside it. -The floor in the storage room off of the upstairs kitchen where soda syrup has leaked. -Any other area of the facility not identified here, where food/dust/dirt accumulation is observed.

Physical facilities not maintained in good repair. Observed the floor in the kitchen is no longer smooth and easily cleanable in areas where sections of sealed concrete are missing. Repair or replace floor.

• Jimmy John’s, 1888 W. Main St., Troy — Repeat: The prep sink plumbing (fixture) was observed in disrepair – water was only able to be turned off/on using the water line valves beneath the sink. Have the plumbing repaired or replaced.

• Clopay Corp., 1400 W. Market St., Troy — Repeat: Improper method for cooling TCS foods. Observed one small pan of cooked buffalo chicken and one small pan of cooked sausage patties cooling in the reach-in cooler in the back storage room. Both pans of food were covered in plastic wrap, and stacked on top of each other. Internal temperatures were observed at 110°F to 115°F. PIC stated they had just put these two items in the unit around 20 minutes ago. Also, observed once pan of refried beans and one pan of taco meat sitting on the prep table in the kitchen with plastic wrap over the top. Ensure that TCS foods are cooled from 135°F to 70°F in two hours and from 70°F to 41°F or below in the next four hours. Ensure that TCS foods are rapidly cooled by leaving containers vented, and separating them in the coolers so they are not stacked on top of each other

Repeat: Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed cutting boards throughout the facility (on prep top cooler, on shelving by three-compartment sink) were scratched, scored, chipped, and stained and no longer smooth and easily cleanable. Replace or resurface.

• Domino’s Pizza, 937 W. Main St., Troy — The shelving unit adjacent to the three-compartment sink and then the supports to the shelving unit above the three-compartment sink were observed with rust build-up. Replace shelving unit and supports.

Repeat: The pizza prep reach-in cooler door frames were observed with cracks. Replace or repair.

The shelving units in the front kitchen area and then the stainless one adjacent to the three-compartment sink were observed with dust and food debris build-up. Remove items and clean.

At the time of inspection, there was not an individual on hand who had a Level One Food Safety Certificate. Obtain certification or bring in certificate. Have an individual on hand at all times who is at least Level One Food Safety Certified.

• Circle K, 1901 W. Main St., Troy — In the donut case, trays of cookies were observed without their common name and ingredient list. Also, the pink icing ring donuts with sprinkles were observed without a common name. Ensure all items in the donut case have common names and ingredient/allergen lists. Also, ensure the common name on the tray matches the common name on the ingredient list.

Cooking and serving utensils in the top drawer beneath the convection oven were observed scattered. Store utensils with the handles facing one way and accessible.

The cabinet door surfaces beneath the third coffee machine were observed in disrepair. To allow for proper cleaning, replace the door surfaces.

• Clare Bridge of Troy, 81 S. Stanfield Road, Troy — The reach-in cooler door gasket was observed in disrepair. Replace.

Repeat: The table adjacent to the dish machine was observed not properly sealed to the adjacent wall. Also, existing caulking was observed with a black, mold-like substance build-up. Recaulk the table adjacent to the dishmachine.

• Market Fresh Foods, 301 Troy Pike, Covington — No violations were documented at the time of inspection.
