Dec. 6

• Tim Horton’s, 700 W. Main St., Troy: Standard Inspection.

Corrected During Inspection; Critical: Equipment and utensils contacting TCS food not cleaned every four hours. Spoke with person-in-charge (PIC) and determined espresso bells are not being cleaned every four (4) hours. PIC stated the bells would be cleaned, rinced, and sanitized every four (4) hours moving forward.

Non-food contact surface(s) not cleaned at the required frequency. Observed ice droplet build-up in the back of the walk-in freezer on the ceiling. Deice and clean to remove accumulation of ice.

Comments: Covered procedure for cleaning espresso bells every four (4) hours by washing, rinsing, sanitizing. Person-in-charge stated they would start doing this immediately.

• McDonalds Tipp City, 127 S. Garber Dr., Tipp City: Standard/Critical Control Point Inspection.

Corrected During Inspection; Critical; Repeat: Food employee(s) did not wash hands when required. Observed food employee touching clothing garment, then dirty dishes, then returning to the prep line without washing hands. Upon making PIC aware the food employee was instructed to wash hands and don gloves.

Corrected During Inspection; Critical: Equipment food-contact surfaces or utensils are unclean. Observed nozzles of the frappe machine with syrup build up. At the time of inspection, person in charge (PIC) cleaned and sanitized the nozzles.

Equipment components are not intact, tight, or properly adjusted. Soda machine observed leaking into the cabinet below in the lobby.

Non-food contact surface(s) not cleaned at the required frequency. Observed food debris in food draws in refrigeration units. Observed food debris in reach in units, underside of ice-cream dispenser and outside of McFlurry mixer.

Water treatment device or backflow preventer not inspected/serviced with documentation as required. Backflow inspection tag stated the device was tested in May 2020.

Repeat: Facility not maintained clean. Observed food debris under the 3 compartment sink.

• Outback Steakhouse, 1801 Towne Park, Troy: Standard/Critical Control Point Inspection.

Corrected During Inspection; Critical: Food employee(s) did not wash hands when required. Observed food employee touch their mask, then phone, then beverage before attempting to don gloves without washing their hands first. Upon making the employee aware, they washed their hands and then donned their gloves.

No soap at handwashing sink(s). Observed soap missing at the handwashing sink in the dish area.

Corrected During Inspection; Critical; Repeat: Equipment food-contact surfaces or utensils are unclean. Observed dirty utensils stored with clean ones hanging on the wall as well as a dirty knife in the knife holder. After making the person in charge (PIC) aware, the dirty utensils and knife were removed from their respective areas.

Corrected During Inspection; Critical: Ready-to-eat, TCS food not properly discarded when required. Observed the following items past their disposition: blue cheese (11/30), orange marmalade (12/4), whipped cream (11/25), chili slices (11/26), cabbage (11/19), basil (11/13), and bacon mustard (11/30). All of the aforementioned items were discarded upon letting the PIC know.

Corrected During Inspection; Critical: Working containers of poisonous or toxic materials not properly labeled. Observed spray bottle with unknown liquid, PIC discarded after making them aware.

Observed single-use carry out sauce containers improperly stored in a bin contaminated with food splatter.

Repeat: Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed small cutting boards throughout the facility deeply scored, scratched, and discolored.

Improperly cleaned storage area for refuse, recyclables, or returnables. Observed trash and debris in the dumpster enclosure.

Repeat: Physical facilities not maintained in good repair. Observed inadequate or missing grout throughout the facility, especially in the dish area.

Comments: At the time of inspection, PIC was made aware that general cleaning and detailing should be performed throughout the entire facility.

During time of inspection, shrimp thawing in the 3 bay compartment was observed. Educated PIC that if the 3 bay were to be used for thawing purposes it should be cleaned and sanitized before and after thawing of the product. The PIC stated that the 3 bay compartment was not being used for washing at this time.

• Little Caesars, 1408 W. Main St., Troy: Follow-Up Inspection.

Repeat: Non-food contact surface(s) not cleaned at the required frequency. Observed shelving throughout the facility with food residue and build up.

Repeat: Plumbing system not properly maintained or repaired. Grease trap lid not secure, observed the lid lifting. Grease trap emitting a smell. Pumping of the grease trap may be needed to mitigate smell.

Repeat: Facility not maintained clean. Floors and walls throughout the facility were observed unclean with food debris and residue. Walls in the dish room observed with what looks like mold growth.

Repeat: Physical facilities not maintained in good repair. The cove base near the grease trap is loose/broken and no longer secure to the wall. Repair to facilitate easier cleaning.

The following violation(s) have been corrected since the last inspection:

Controlling pests: Presence of live insects. Gnats were observed in the dish room and near the back hand sink.

Comments: Very few gnats were observed at the time of the follow up inspection. Person in charge (PIC) showed MCPH a pest control invoice dated 11/16 for treatment of gnats.

MCPH advised to eliminate standing water gnats bread in by fixing the grout and tiles in the dish room.

Dec. 7

• Hickory River Smokehouse, 135 S. Garber Dr., Tipp City: Follow-Up Inspection.

Repeat: Non-food contact surface(s) not cleaned at the required frequency. Observed heavy grease build up underneath deep friers on the equipment. Clean under friers to maintain equipment.

Repeat: Ventilation system not maintained. Hood filters above deep friers observed to have grease build up resulting in drip potential. Clean hoods to prevent potential grease drips onto food and/or food contact surfaces.

The following violation(s) have been corrected since the last inspection:

Cleanliness of equipment food-contact surfaces and utensils: Equipment food-contact surfaces or utensils are unclean. Observed mold-like substance in ice machine near walk-in cooler. Ensure food contact surfaces are cleaned as frequently as necessary to preclude accumulations.

Comments: Received photos of cleaned ice machine 12-07-21 via phone.

• Taco Bell #23736, 1420 W. Main St., Troy: Standard/Critical Control Point Inspection.

No soap at handwashing sink. Handsoap dispenser not functioning. Ensure soap is available for handwashing and the dispenser is functioning.

Comments: Facility observed clean at time of inspection.
