Dec. 28

• Hale House, PO Box 9, West Milton: Standard Inspection.

No violations were documented at the time of inspection.

Comments:

Food was not actively being prepped at the time of inspection.

• Applebees Neighborhood Grill & Bar, 1759 W. Main St., Troy: Standard/Critical Control Point Inspection.

Corrected during inspection; critical: Equipment food-contact surfaces or utensils are unclean. Observed a mold like substance in the nozzle of the soda gun in the bar area. Upon informing PIC the nozzle was removed and cleaned.

Corrected during inspection; critical; repeat; Presence of live insects, rodents, and other pests. At time of inspection observed gnats present in the dish and bar area. Floor drains were observed with extreme build up that could be contributing to the presence of pest.

Repeat: Equipment components are not intact, tight or properly adjusted. Observed excessive ice build up in the walk-in freezer do to malfunctioning freezer door.

Repeat: Non-food contact surfaces of equipment are unclean. Observed reach in coolers and freezer on the line with food debris build up on the inside.

Repeat: Facility not maintained clean. The following areas need more attention: 1. Behind fryer line 2. Floors in freezer 3. Floors and walls below server station 4. Floor drains throughout facility 5. Floors under equipment throughout facility

Comments: At time of inspection, facility shows improvement since last inspection. Continue deep cleaning throughout in addition to the areas identified during the inspection.

Cleaning frequency behind and under equipment still needs improvement. Floor drains throughout the facility were observed with buildup and need detailed.

At time of inspection there was still a gnat presence, however the infestation was less severe than previous inspections. PIC provided pest control statements dated for 12-28-21. Discussed with PIC the need to follow recommendations given by the pest control company.

• Burger King Troy, 1829 W. Main St., Troy: Follow-Up Inspection.

Repeat: Equipment and utensils are not being air dried. Observed food containers stacked together while still wet (wet nesting).

Repeat: Irreversible registering temperature indicator not readily accessible for hot water mechanical warewashing operations. Facility did not have irreversible heat test strips to test the dish machine at the time of inspection.

Repeat: Non-food contact surfaces of equipment are unclean. Observed food debris on the outside of hot holding drawers.

Repeat: Facility not maintained clean. Floors throughout the facility were observed unclean with food debris and grease build up.

The following violation(s) have been corrected since the last inspection:

Cleanliness of equipment food-contact surfaces and utensils: Equipment food-contact surfaces or utensils are unclean. Observed build-up inside the ice machine bin where ice is held.

Comments:

Pictures of the ice machine taken apart and cleaned were received by MCPH.

• Arby’s #642, 903 W. Main St., Troy: Follow-Up Inspection.

Repeat: No soap at handwashing sink(s). Observed no soap at the hand washing sink near the dish area.

Repeat: Equipment components are not intact, tight or properly adjusted. Observed ice build up around the door of walk-in freezer.

Repeat: Non-food contact surfaces of equipment are unclean. Observed food debris build up on the racks in the walk-in cooler.

Repeat: Facility not maintained clean. Observed food debris build up under and behind equipment. The following areas need specific attention: 1. Under and behind deep fryer 2. Behind BIB rack 3. Floors in walk-in freezer and cooler

The following violation(s) have been corrected since the last inspection:

Cleanliness of equipment food-contact surfaces and utensils: Equipment food-contact surfaces or utensils are unclean. Observed a mold like substance inside the bin of the ice machines in the dinning room and drive thru.

Repairing: Physical facilities not maintained in good repair. Observed the reach in cooler on the line in disrepair and holding a temperature above 50 degrees F.

Comments: At the follow-up inspection, both the drive thru ice machine and the dining room ice machine were observed completely clean, free of any mold-like build up. Additionally, the cold-holding unit was repaired and observed holding temperatures of 41F or below.

• Bob Evans Restaurants LLC #257, 1749 W. Main St., Troy: Follow-Up Inspection.

Repeat: No towels or drying device at the handwashing sink(s). Observed no paper-towels at the hand-sink nearest to the capuccino machine. Also, in the employee bathroom, hand towels were being stored on the floor and thus contaminated. Ensure that the dispensers are properly stocked so food employees can dry their hands adequately after washing.

Repeat: Irreversible registering temperature indicator not readily accessible for hot water mechanical warewashing operations.

Repeat: Non-food contact surface(s) not cleaned at the required frequency. Observed microwaves on the cook-line encrusted in food debris and observed the capuccino machine not maintained clean.

Repeat: Facility not maintained clean. Observed floors, walls, and ceilings throughout the facility covered with food residue and debris.

Comments: Pictures received showing the critical violation of the ice machine was corrected.
