March 26

• Covington Care Center, 75 Mote Drive, Covington — Non-food contact surfaces of equipment are unclean. The undercounter stainless surfaces of the steam line were observed needing to be cleaned at the time of inspection. Ensure these surfaces are clean to sight and touch.

Physical facilities not maintained in good repair. The fiber reinforced paneling (FRP) behind the ice machine was observed removed entirely from the wall. Repair or replace. The floors of the walk-in cooler (WIC) were observed rusted out and in disrepair. Repair WIC floors to ensure they are smooth and easily cleanable.

Corrected during inspection; critical: Handwashing sink is being used for purposes other than handwashing. Observed the soda dispensing gun being stored in the drink station hand sink. Hand sinks are to be used for hand washing only, not to peform other tasks such as rinsing the soda dispensing gun. PIC removed the soda spray gun from the hand sink at the time of inspection.

Critical: Insufficient air gap between the flood rim and the water supply inlet. The ice machine drain line was observed without a proper air-gap. The PIC stated the plumbing that was previously secured with an air-gap had recently come loose. Re-attach the plumbing line with a proper air gap to protect against backflow contamination into the system.

Repeat: Facility not maintained clean. Observed the floors underneath and behind equipment throughout the food service with a build-up of debris and other residues. The PIC stated a deep cleaning is scheduled to occur somtime next week. Enhance the cleaning frequency of the physical facilities to prevent the sort of observed accumulations.

• Old Mason Winery and Vineyard Inc., 4199 S. Iddings Road, West Milton — No violations were documented at the time of inspection.

March 29

• He5 Asian Bistro, 121 E. Staunton Road, Troy — Facility not maintained clean. Behind the grill/wok line as well as the deep fryers, observed a build-up of food debris and other residues. Ensure equipment is being pulled out and cleaning is taking place frequently enough to prevent these sort of accumulations.

Corrected during inspection; critical; repeat: Refrigerated, ready-to-eat, TCS foods not properly date marked. In the reach-in prep cooler, observed cooked wontons without a proper date label. In the walk-in cooler unit, observed cooked wontons and cooked chicken also without date marks. Upon informing the PIC, these items were properly dated with appropriate dates of disposition.

Corrected during inspection: Non-food contact surfaces of equipment are unclean. The reach-in handle to the prep unit was observed soiled with food residue build-up. Enhance the cleaning of this non-food contact surface to ensure it is kept clean to sight and touch.

Repeat: Ventilation system not maintained. The hood filters were observed with build-ups of dust and grease deposits. The PIC stated the hoods are scheduled to be cleaned on 4/2 at 10pm.

• China Moon, 1055 S. Miami St., West Milton — Corrected during inspection; critical; repeat: Refrigerated, ready-to-eat, TCS foods not properly date marked. In the walk-in cooler, observed cut cabbage, cooked chicken and wontons without proper date marks. Upon informing the PIC, these items were all able to be properly date marked at the time of inspection with their appropriate date of disposition.

Corrected during inspection; critical:

Food not properly protected from contamination by separation, packaging, and segregation. In the reach-in prep unit, observed one set of tongs being used for both raw chicken and raw beef. Upon informing the PIC, another set of tongs was placed in the unit to prevent the potential for cross-contamination.

In the walk-in cooler, observed a container of raw chicken labeled for personal use being stored above raw shell eggs and ready-to-eat food items. Upon informing the PIC, the raw chicken was moved to the bottom shelf.

March 30

• Homestead Golf Course and Pop House, 5327 Worley Road, Tipp City — No violations were documented at the time of inspection.

• Outback Steakhouse, 1801 Towne Park, Troy — Equipment and utensils are not being air dried. Observed wet bowls being stored nested inside one another and not properly air dried.

Corrected during inspection; critical; repeat: Equipment food-contact surfaces or utensils are unclean. Multiple food utensils and containers were observed with food debris and residue on the clean utensil rack. Upon making the PIC aware, the items were put in the dirty dish area.

Corrected during inspection; critical:

TCS foods not being cold held at the proper temperature. Cut cherry tomatoes in the salad prep unit were observed holding 56 degrees F. Upon informing the station worker, the cherry tomatoes were able to be cooled and retempted holding 41 degrees F.

Improper use of time as a public health control (4 hours). Butter and breading batter were being held at room temperature for 4 hours and were not properly time stamped. Upon making the PIC aware, the items were time marked for when they were pulled out.

Critical: Presence of live insects, rodents, and other pests. In the bar area, observed several live gnats. Ensure pest control is contacted and the issue is remediated. Remove standing water and the slight odor that was present beneath the bar area and near drains.

Corrected during inspection; repeat: Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed multiple moveable cutting blocks for food prep that were severely scratched and scored. Upon informing the PIC, these cutting surfaces were voluntarily pulled and discarded.

Repeat:

Non-food contact surfaces of equipment are unclean. The under counter prep cooler along the grill line was observed with food debris and residue build up. The prep top cooler for shrimp was observed with a spill in the bottom reach in area.

Physical facilities not maintained in good repair. Observed missing or inadequate grout throughout the facility, especially in the following areas: 1. dry storage 2. server area 3. dish-pit area

Facility not maintained clean. Floors throughout the kitchen, coolers, dish area, and dry storage were observed with food debris and build up. Food/oil splatter was observed on the ceiling and wall above the potato prep table.

• Staunton Country Store, 17 S. State Route 202, Troy — Accumulation of refuse, recyclables, or returnables. Behind the shed in the parking lot, bucket lids, drink racks and other equipment were observed piled up.

Corrected during inspection; critical:

Handwashing sink is being used for purposes other than handwashing.Observed one side of the hand sink being used for rinsing tomatoes. Discussed with the PIC the hand sink must only be used for hand washing.

Ready-to-eat, TCS food not properly discarded when required. Observed multiple premade sandwiches date marked for 3/28 and 3/29 at the time of inspection. Upon making the PIC aware, the sandwiches were all discarded.

Corrected during inspection; critical; repeat: Refrigerated, ready-to-eat, TCS foods not properly date marked. Observed coney chili sauce date marked for 8 days. Upon making the PIC aware, the out date was changed to meet the 7 day date marking rule.

Repeat:

RFE does not comply with Level One Certification requirements. The facility did not have a level one (food handler) certified individual at the time of inspection.

Facility does not have an employee with manager certification in food protection.

Non-food contact surface(s) not cleaned at the required frequency. The following surfaces were observed with a build-up of food debris and other residues at the time of inspection: 1. Food and bread carts/racks 2. The lowboy stainless surfaces of the prep line 3. Exterior of reach-in cooler units and their handles Enhance the cleaning frequency of these surfaces

Ensure the following areas are being cleaned more frequently and thoroughly to prevent the sort of observed accumulations of food debris, dust, dirt and other residues: 1. Along the walls and tile cove base finish areas 2. Underneath and behind equipment 3. The dishpit area 4. Under/behind the makeup stainless prep table 5. Behind/under the fryers.
